Please use this form to report any concerns you have about a student. Please remember if this concern involves hurting themselves or someone else, do not use this form, contact an RD and/or Perm staff immediately and do not leave the student alone. Someone will follow up with this report and contact you within 24 hours. If you have not heard from someone about the concern and/or the concern gets worse, please contact someone in person.
