MOTOR POOL RESERVATION FORM


Please use this form to request a vehicle from the Tusculum University Motor Pool.

If a conflict exists between the time you request and a previously scheduled reservation, you will be notified of the conflict. You will also receive a confirmation once the reservation has been made.

No reservation is confirmed until you receive a confirmation via email from the Facilities Department.

  • All drivers must have an approved Drivers License on file in Human Resources prior to driving any University vehicle
  • There will be a minimum fee of $50 if vehicle(s) are not free of debris and trash upon return, examples (food, bottles, fast food items, gum wrappers, clothing, etc.)
  • Vehicle(s) are to be returned with full tank of gas. $75 charge will apply if not returned with full tank.
  • Report accidents/mechanical problems to 423-636-7319 during working hours and 423-470-2710 after working hours.
  • In the event of vehicle accident always obtain a police report.
  • A 24hr notice is required for cancellation; $100 charge will apply if no cancellation is given.
  • Tusculum University is not responsible for personal belongings left in vehicles.
